Team Have Dreams Chicago Marathon 2013 Commitment Form

Please fill out this form IF YOU WOULD LIKE TO JOIN TEAM HAVE DREAMS for the 2013 Bank of America Chicago Marathon.  Each registered team member is required to raise a mandatory minimum of $600.  If you are seeking a Post-Close Entry after registration closes, the fundraising commitment will be $1,000. Team members taking advantage of the CARA training and membership are required to raise an additional $150.
